What is the procedure for requesting a new patient appointment?

Call office at 732 444 8802 to request appointment.

How do I request a refill for medication?
If you need a refill of your medication, email us at info@swc-nj.com or call the office directly at 732-444-8802. Medications will be filled within 24 hours unless it is a weekend or holiday in which it will take up to 72 hours.

We do not respond to requests from your pharmacies.

What if I need external paperwork filled out?
Please contact our office to let us know what type of paperwork needs to be filled out. In certain circumstances an office visit may be required.

Does our office prescribe Emotional support animals?
No, we do not.

Why does Shrewsbury Wellness Center require a credit card on file?
Shrewsbury Wellness Center has a credit card on file policy. Co-insurance payments, deductibles, co-payments, No Show/Late Cancellation fees will be charged to this payment method unless another form is received at the time of the scheduled appointment or prior arrangements have been made with our office.

Patient Financial and Scheduling Policies

  • Appointments not canceled at least 24 hours in advance will incur a $150 late cancellation/no-show fee.

  • Psychiatric Evaluations not canceled at least 48 hours in advance will incur a $250 late cancellation/no-show fee.

  • All credit card payments are subject to a 3% convenience/processing fee.
